I-693a in Sealed envelope

Today we got Fernanda's DS-3025 vaccination worksheet transferred to the I-693a by a Civil Surgeon. He gave us the I-693a in a sealed envelope. My question is do we submit the sealed envelope with AOS petition or do we open it, make a copy and send the copy with the AOS application?

It needs to be sealed and unless you can get the envelope not to look like it was tampered with I wouldn't risk it, unless it's just in a blank envelope. My wife's doc gave us a copy, sealed it, and stamped the seal, like a wax seal would be done.

Ask the civil surgeon to give you a copy. I have never heard of them not doing this before. Don't open the sealed envelope.

When I came back to the doctor, I asked for a copy. They are supposed to get one too, so you can ask for a copy of that report.

We were given a copy and sent the sealed envelope with the rest of the AOS documents today.

The doctor didn't give me a copy either... I wasn't too worried about it seeing as the receptionist pretty much ignored the DS form and went by my own immunization record. If they said they lost it I am sure a copy wouldn't work anyway... so would need to get it done again. Just send the sealed envelope and if you're really anxious to see what you already know then call them and ask for a copy.

Everyone's right DO NOT OPEN IT! The Civil Surgeon explained that it's sealed only for the purpose of the government knowing that a Civil Surgeon actually filled it out, because the actual form can be filled out by anyone. The fact that it's sealed in a certain way is a guarantee to the government that a doctor did check fill it out. There's nothing secret in the envelope! A copy of it will do you no good, if it gets lost and they send you a RFE for it, you will have to go back to the civil surgeon and get it sealed in the envelope again.

I really don't think it matters if it is in a sealed envelope. A lot of us were approved by just sending in a copy of our DS-3025. Nothing sealed and no special forms from a civil surgeon.

The civil surgeon is supposed to give you a copy. Call and ask them to send you a copy (though you probably really don't need it and it's pretty boring) if you are worried.

I too received my copy from the Civil Surgeon in a sealed envelope with a type written note on the envelope saying DO NOT OPEN. I asked them for a copy, and they replied that if for whatever reason, USCIS lost the envelope, I could call them back and they would reissue another in a sealed envelope again.

Yes, some people who apply for AOS and don't send in a Vaccination Supplement do get approved. However, if you are going to send one in, or are RFEed for it then it needs to be in a sealed envelope. This is so that they know a Civil Surgeon filled it in. As mentioned before, anyone could fill it in and sign it, so this is a way to ensure it was done properly.
