my husband and I took the chance and just sent in his DS-3025 vacc. worksheet. we were very lucky and flew through the AOS process. just today i came across this civil surgeon who will transfer the info. for only $10. i heard that you mail him the vacc. worksheet DS-3025 and they do there thing and make a copy for your records and mail it all back to u. best of luck..... here's the info....>>>>

Jus to let you know that I too used this civil surgeon, and he doesn't mind where in the US you are - it is all done by post. They are really friendly, helpful and professional!!! A great find.

Havent posted here for a while. Getting my AOS form and documents ready to be sent off.

Can i just send my DS-3025 worksheet to this doctor along with a check for $10 to have him transfer my vaccination details over to the I-693 vaccination supplement or will i need to call this doctor first??

any help would be appreciated.

Do we have to take the vaccination form that has already been filled out by a UK civil surgeon to a US Civil Surgeon just so he can copy the UK info into the I-693A or do can we just submit his UK Vaccination record in lieu of an I-693A?

My (then) fiancee's DS-3025 was sealed inside the K-1 envelope issued by the consulate, so we never had it in our hands. It went straight the CBP officer at Miami.

When we submitted for AOS, we included a brief letter stating that all vaccinations were completed in the UK prior to the consulate interview, as evidenced by the DS-3025, hence we submitted no I-693A. No RFE was received.

This might be an option if your circumstances are the same as ours.

My Fiancee did not get a copy of the DS-3025. I would assume that I can do the same as you did. We do have a translated copy of her vaccination documents though.

Can anyone see if this will be a problem?

Since my wife had all the necessary vaccinations we sent in the DS-3025 and were approved with no problems.

Would you happen to have a copy of the letter that you sent in lieu of your DS-3025. My husband and I have the same problem. He was given a sealed envelope which was submitted to the cbp officer and we didn't get a copy. It seems we may have to do the same as you. We have an appointment for the I-693A Supplement w/ a doctor, but just to be on the safe side. We'd greatly appreciate your help.
