Do we have to take the vaccination form that has already been filled out by a UK civil surgeon to a US Civil Surgeon just so he can copy the UK info into the I-693A or do can we just submit his UK Vaccination record in lieu of an I-693A?

Some people received RFEs for only submitting the worksheet from their foreign medical and some people don't. I didn't want to risk an RFE so I got the information transcribed to a I-693A by a civil surgeon.

2007-08-23: Green card production ordered!!!

You go to a Civil Surgeon in the US and he or she will transfer the information onto a I-693a. We did this exact thing today. Check around your city as prices will vary widely from one Doctor to another.

my husband and I took the chance and just sent in his DS-3025 vacc. worksheet. we were very lucky and flew through the AOS process. just today i came across this civil surgeon who will transfer the info. for only $10. i heard that you mail him the vacc. worksheet DS-3025 and they do there thing and make a copy for your records and mail it all back to u. best of luck..... here's the info....>>>>

My (then) fiancee's DS-3025 was sealed inside the K-1 envelope issued by the consulate, so we never had it in our hands. It went straight the CBP officer at Miami.

When we submitted for AOS, we included a brief letter stating that all vaccinations were completed in the UK prior to the consulate interview, as evidenced by the DS-3025, hence we submitted no I-693A. No RFE was received.

This might be an option if your circumstances are the same as ours.

Yes, you can apply for SSN after your marriage but you should wait for about 2 weeks after enter the US to do so otherwise the SSA won't be able to issue you a SSN. And don't wait until close to the expiration of your I-94, some offices will give you a hard time and won't issue a SSN until you got your AOS approved, which can take a while! Good luck!

I am not sure why the doctor didn't indicate if you had all your vaccinations or not. But, it should be available to see if above they say it's complete or incomplete. I am sure it's not anything to worry too much about.

the doctor i gone to in egypt he gaave me the vaccnation paper and told me u going need that at ur green card papers in USA dont loose it and dont give it to the embassy, they just need the sealed envolpe
