That won't fly, the husband is a USC and he is staying home.     And the mother doesn't need a consent letter for the Philippines  ;)

With obvious changes the form can work as an affidavit for single parent travel document. OP needs to remove references to passport and the document will serve their purposes.

Nathan -

How did the travel for your family go?   Im in a similar boat....

Me, my wife (K1) and my son (12yo) will be going end of June....

We all have our passports.

My son's mother signed off so he could obtain his passport in absentia... she travels around alot so its hard to get ahold of her but eventually we did.

Is there anything we will need for the 3 of us to enter?    Did they ask your wife for the paperwork of your daughter?

We will be flying Asiana - not sure that matters or not.

Make sure you bring your marriage certificate with you and PRESENT IT TO IMMIGRATION UPON ENTRY and if she has her green card then present that as well. I can not say that again because it is important, otherwise than that you will be fine. They never asked for paperwork for our daughter at all when we went.

Are you all flying back together or will your asawa stay back to spend some time with family?

To be clear... my son is from a previous marriage.   His mother had to sign the application for his passport and have it notarized along with copies of her ID since she was not present when applying for his passport (both parents need to show up for a minor childs passport application)

The 3 of us, Me, my son and my wife  (not my sons mother)   will be traveling together and leaving to return to the U.S. together.   So I suppose I can bring the marriage certificate.

My main concern is that they will not ask for documentation proving that I have my son's mother's permission to travel like I had to get with the passport application.

These are the only requirements that I see on the PH Immigration site:

9. What are arrival requirements for Philippine-destined alien tourists?

9.1. For aliens with diplomatic or bilateral agreements with the Philippines:

  1. Unexpired passport;
  2. Unexpired entry visa; and
  3. Validly-issued return ticket.

I read your other post and I was commenting primarily on your wife and not your son but I will comment on that now. When I take my two older kids with us to the Philippines I will have a letter from my x-wife allowing me to take them there just in case. So you should get one too just to cover your butt.

i didnt think about that. Glad this was brought up. she will be most likely laying over in China somewhere.not sure where exactly yet.

I had this "permission" stipulated in my divorce decree so I don't have to seek permission every time I want to leave the country with my daughter

YMMV

I keep a copy of it on my email of my divorce and my custody case.

Exactly!    Better safe than sorry.

I had to actually get a certified copy of my divorce decree a couple weeks ago so I can add my wife to DEERS, they still have my x-wife as my current wife.
