I-90 Renewal "Where did you apply" ??

Hello all.  I am beginning the process of renewing my PR card (Green Card) it will expire in 6 months.  I am stumped on question 3 of the form.

it asks,

Where did you apply for your immigrant visa or adjustment of status?

and 

Where was your immigrant visa or adjustment of status issued?

I can see on my card it was approved in LIN (Nebraska Service Center)

However I can not remember where I applied?  I suppose this would be where I mailed it to, but I haven't a clue.  Is there a way to find this information out?

They are asking for the city/state, or city/country where you were living in at the time of your initial filing.

They are asking for the city/state, or city/country you were living in at the time it was approved.

Is it just that simple??   I was certain I needed to add the USCIS center at least for the second part of the question

Based on the city/state you enter in for that question----they will know which USCIS center it was sent to.

Did you interview at a consulate in canada or did you adjust your status?

For example both answers for me would be Montreal since that is where i went for my CR1 visa interview.  

For you, it will either be a service center or local office if you adjusted your status vs a spousal visa.

NikLR, thank you for responding.  

Once in the United States we applied to adjust our status.  It was all done for us through my husbands work.  Before the PR was granted I was here on an H-4 visa.
