Family disagreement or family rejection will not be the reason for them to deny your visa. However, US embassy in India is very strict. Furthermore, you have few red flags.

1) She is older than you (to overcome this, provide more bonafide relationship proof)

2) Your parents are not aware of it (to overcome this, you need to tell your parents about it. To be honest, I'm so afraid to tell my parents about my bf (now my fiance) before too. I feel like they will reject him because we had a different religion. However, I know that it will be a red flag if I keep it hidden and of course, my family is important for me. I don't want to do something in secret. I end up telling them. As what I expected, they rejected it and ask me to choose between them or my fiance. I choose my fiance and I need to go out from the house immediately after that. It is not easy. However, that is the best thing to do rather than keep things hidden. 

3) Lack of time in person. (To overcome this, you guys need to meet more. Since you said that she willing to come and have one or two visits, it will help. But, again, US embassy in India is very strict. I will suggest two more visits if I'm you

Aside from that for your query (number 2 & 3)

Query 2: It is okay to not have a picture of exchange the ring. Just provide other supporting the receipt of the ring, a picture of the ring itself. I don't even have an engagement ring yet. But, I know that it is not a big deal (not sure about US embassy in India)

 

Query 3 : You need to tell them the truth. However, first, you need to tell your parents first. Hiding it from your parents is a huge red flag. I will be having my interview on March 8, if the embassy asking me about my parents' approval, I will answer like this "They are not happy about the engagement. They gave me two option whether forget him and I will be able to stay at home or choose him, but I need to get out from the house and they don't want to know anything about me at all. I chose option 2 since I love him so much and I know that by the time pass by, my parents will be able to accept me. It is a huge decision to make and a huge risk to take. However, I believe in my guts and I know that he is my soulmate and I want to have a family with him. In the same time, of course, we will still be working on to make our relationship with my parents better in the future"

They are just preventing fraudulent petitions from getting an approved Visa, and protecting their citizens from fiancé(e)s who are only using the citizens for green cards.

Yeah there is a huge difference between only meeting once in the past two years and only meeting once ever. Big red flag. How well do you really know someone you've only once?? You need to date for a while to get to know each other.

There are no right or wrong answers. Having lived in India, dating is a relatively newer concept; not everyone dates, not in small towns. Open dating is more prevalent in the larger cities, that too among an upper-middle class subset. That being said, hiding this from family is a huge red flag. Indian families frown upon a son marrying someone older, and COs in India are trained to understand local customs. Young men are known to troll the internet/facebook to lure American women into online relationships. Not putting OP in that category, but COs look for that.

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.

You leave out a lot that will be asked.

 

How you met and when. 

 

I would personally be suspicious of two people who don't seem to know each other wanting to get married secretly.  For your interview, you'll need a lot to back it up.  You have the bare minimum so far, so it may be worth taking time to build a credible relationship. From your description you sound like strangers who just met.

If she wants to visit twice, can't she visit BEFORE it gets approved and then BEFORE the interview? You've met only 1 time, and if she just goes right before the interview, it will look like she is doing it for the interview and not to meet you and your friends, family.  Also, if it is not approve, would it mean she would not visit you any more? Another odd choice.

 

I know you didn't tell your parents, but she has to meet some of your family and friends. Otherwise, it looks like you are meeting her family and friends, and she is not meeting anyone from your side. It will be odd.

You have a big red flag :

 

1. your parents don't know about her. Indian Consulate and CO know the local custom. 

2. At present, Age difference is not too big issue (even thou it is in Indian consulate).

 

If you guys really love each other then speak up with your folks.

 

What you need?

Having her being involved with your family & friends and documenting that family moment with photos. 

Affidavits from your family and friends supporting your relationship

Maybe couple of more visit and her time with you and your family and friends

 

You are marrying someone. It would be crazy of her not to meet your family and your family not meeting her. It's not something you can hide. It's marriage. If I were a CO, I would deny you a visa in your current situation. 

 

sorry, I am being judgemental but hey we all are 'coz we provide the suggestion without any BS.
