Bringing Mother of K1 Visa resident

I hate to bring this to the table but Spain is a great place to be for medical care across the board.  And not only that she is surrounded by familiarity and the place where she grew up.  Add to the difficulty level an elderly person who's main language is not English.  This can affect her comfort level and treatment here.  It doesn't make any sense bringing her here to the USA at all.  OP does his Mom really want to come to the USA, or is it because hubby insists on having mom here?  If it is the latter you need to reexamine why you are thinking about doing this in the first place.  Bringing her to the USA isn't a magic bullet.

Very true, people often don’t think of the psychological shock of taking someone away from everything they know when they are already old and frail. Not sure about OP’s MIL but a number of my Spanish friends tell me many of their parents barely speak English, having grown up in a time when Spain was fairly isolated. OP, does MIL want to basically be entirely dependent on your family for everything, money, essentially being her entire support network, etc? Does she have friends or other family where you live? If her English is poor she will need someone with her on every doctor visit, etc too.
