FORM I-90

Good afternoon all, I am filling out Form I-90 application to replace permanent resident card.  The resident card expired and I need to renew it.  However, my question is to Part. 3 "Processing Information," questions 1 & 2.  Location where you applied for  an immigrant visa or adjustment of status.  The expired resident card says MSC or currently called The National Benefits Center.  Is that the answer for the locations for question 1 & 2?  I deeply apologize if this question has been asked.  Thank you in advance for all the responses.

Is this to renew an existing 10 year greencard? Or was it a 2 year conditional greencard? The reason I ask is because if it's the 2yr conditional greencard, you have to go through removal of conditions via an I-751and NOT the I-90. If it is to renew your existing 10year greencard, then see below:

Part 3. Processing Information

 

Item Number 1. Location where you applied for an immigrant visa or adjustment of status. List the location of the U.S. Embassy, U.S. Consulate, or USCIS office where you filed your application for an immigrant visa or adjustment of status.

 

Item Number 2. Location where your immigrant visa was issued or USCIS office where you were granted adjustment of status. List the location of the U.S. Embassy, U.S. Consulate, or USCIS office where you were issued an immigrant visa or granted lawful permanent resident status.
