the DS3025 is definitely in the envelope from embassy and Fullerton doesn’t provide a medical copy now. So there’s no way for us to keep the ds3025 copy or even original file.

Ahh I see... so what is the medical document that is needed for adjustment of status, are you able to kindly enlighten me? What I have researched is that we need some kind of medical documentation that we needed to submit for AOS and I assumed I can submit the copy of Vaccination Document Worksheet that Fullerton has sent me.

 

I also see the 'DS-3025' form printed at the bottom left hand side of the page so I was assuming that was the medical documentation

Ds3025 can be requested from Fullerton..

i honestly think that this Ds3025 is also not needed to submit since this is included/together with the medical records (for sure! )..because what is the point of submitting the same vaccine record?... Anyway.. most of the people that i chatted with submitted it together with their aos.. So i guess ill just do the same when the time comes.. Or maybe ill have more knowledge by that time that i can be sure that it's really not needed..

They called me to get the picture of my record because I did the influenza in December and they didn’t write it down.. so they neeeded the proof thy I did it with the invoice and name of the clinic and so on..

If you got the ds3025 then that’s great because when I asked. They didn’t provide me the copy.

but anyway it’s optional in AOS file. And if you live in a state that take more than 1 year to get the interview, you will meet to do the health check again anyways.

ahhh that fast? my medical appointment is on march 1st i tried to call several times to see any available slot in Feb but apparently they are fully booked. in that case i shall reply packet 3 maybe next week :)

Yeah.. That was her problem.. She scheduled for medical on 19mar.. After she sent her packet 3 she was scheduledfor interview on march 5..so fortunately Fullerton accomodate her medical today..

yeah i have called Fullerton several times but they told me Feb slots are all taken. Another question, what are the documents you guys replied for packet 3? do i need to include evidence of relationship? so glad to hear you guys interview is coming real soon! all the best!

Try to call them every day or every other day, there will be available slot if someone cancelled, my medical moved from mar19 to mar12, not bad

 

As far as i know you only need to send back 3 things

oh really? i got reply from the forum earlier that i need to submit all the photocopies of document when i reply for packet 3. Anyway i think i need to estimate 2 weeks time for interview so it should be somewhere after medical. i actually booked my medical way earlier around Feb 8 and i have to reschedule it because i underestimate the delivery for packet 3. then i got reschedule to next available slot which is mar 7. i called again once i received packet 3 and got moved from Mar 7 to Mar 1! will try my luck one more time end of this week or i shall wait until Mar 1st. Oh btw is it compulsory to have COC send directly to Embassy? i got my COC right before i get my packet 3. can i bring it along to interview or reply together with packet 3? i don't want to spend another $55 again

Maybe that someone who replied to you is not from Singapore consulate.. Some are embassy specific.. 

 

According to sg police they dont directly send coc.. (at least not anymore) .. So you'll be fine hand carrying it during interview
