Go to your local health clinic (or hers) and request the vaccination records. It's likely they would be there. If not, vaccinations should come up at the medical part of your immigration process, and when you get that done, you'll get a form saying you have xyz done. When I went through the K1 process, there were a list of vaccines that I needed to have. Its likely the same with your moms case.

Hi guys,

My mom did receive some vaccinations, but can not remember whether there was documentation or the document was lost. So can I explain in this way? How will no documentation affect the application?

Thanks

"All Vaccinations will be taken during medicals before my interview" should be your explanation.
