Greetings!

When my brother changed his name because it was that long. We filed a petition to the court and followed everything from paying the fees, publishing it to the newspapers explaining the cause of change and the date of its court hearing. This is to avoid fraud and may allow someone to object the changes. ( For sure no one will object but without legal notice, you will be unable to get a court order to change your name). After the judge grants the name change and after endorsing to the National Statistics Office, my brothers birth certificate has a remark.

Things we submitted: We just submitted anything that has his new name and most of the documents have notes that stated that his name was changed. This was a long time ago, so I am not sure if there is a new requirement.

1. BIRTH CERTIFICATE WITH REMARK STATING HIS NAME CHANGE.

2. SCHOOL ID WITH HIS NEW NAME.

3. DRIVER'S ID WITH HIS NEW NAME.

4.SCHOOLS TRANSCRIPTS WITH HIS NEW NAME.

5. BANK ACCOUNT WITH HIS NEW NAME. ETC.

 

If I am not mistaken, I think the birth certificate with a remark from NSO is sufficient as it passes the proper way of changing the name that was granted by a judge from the court. Opps! I am sorry my brother is the beneficiary. Well, a proof of name change is the same whether you are the beneficiary or the petitioner. Or some things may be different especially if one has a naturalized citizenship.

I found this site that is related to your topic.This is from the State of California, I guess I assumed that petitioner is from California?. Anyways, if not then I am sure that instructions from other state are available online for guidance.

You can use this to change all your legal documents, including your birth certificate, social security card, and other government-issued identification like your passport or driver’s license.

 

Note: As with my brother he uses the decree to change all his documents.

My Aunt name was changed after her Naturalization but she cant remember any form she submitted ;(
