How is everyone doing this morning? I was looking over the instructions for the AOS filing over the weekend and have a few questions. All help will be appreciated.

1) Does the G-325A form need to be filled just for the non us citizen spouse or for both the petitioner and the beneficiary?

2) Its mentioned that the I-94's both front and back copies should be sent. Is the back really needed, as its blank. Further, on our passport its stapled such that its near impossible to get the complete copy of the back page without removing the staple which I definitely would not take off. What should I do?

3) What is I693-A? I didn't find such a form on the USCIS site. We have a DS-3025 and an X-ray from India. What should we do with these? Do we need a medical examination in the US?

4) We are not required to file any I-485 supplement. Is it safe to assume that no police clearances are thus needed to be sent with the AOS application.

For I-485, only the benefitiary needs to fill out G-325A.

2) Its mentioned that the I-94's both front and back copies should be sent. Is the back really needed, as its blank. Further, on our passport its stapled such that its near impossible to get the complete copy of the back page without removing the staple which I definitely would not take off. What should I do?

Just remove the I-94 from the passport and photocopy both sides, then staple it back in. There's nothing magic about a CBP stapler.

That is the supplement to the I-693 medical form that lists whether the benefitiary has the required vacinations. Please do a search on this website for the many discussions of the process of getting this form.

No police clearance is needed for AOS.
