I'm now divorced.  When my ex left (3 yrs ago), she placed a restraining order on me thinking I may flip out and cause harm to her.  I didn't contest the order (and really didn't think it would matter) since I wanted the marriage to be over with and had no plans to interact with her anyhow.  Now that I've found love again and as I'm on this site, my fiance is overseas and I'm petitioning to get her here so we could marry.  The restraining order has expired and needless to say, I've not had contact with my ex-wife since the day she left 3 yrs ago.

 

Do I have to disclose this information?  How do I respond to Part 3.  Can someone help with answering all part 1-4?  The only other thing in my past is a misdemeanor offense 20 years ago.
