Yes, you have to include form I-864 along with a copy of your husband's W2 and Federal individual income tax return for 2006 (copies of his federal income tax returns for 2005 and 2004 are optional) . If you feel it would help your case, you could also submit paystubs from the last 6 months, a recent bank statement or an employer letter. You don't have to send them a copy of your Social Security card.

Here is my I-485 check list. Note that I'm adjusting from F1 not K1 and that I didn't apply for EAD.

I-485 Package

a. Cover Letter

b. $395 check for Form I-485 and biometric services payable to the Department of Homeland Security

c. Form I-485: Application to Register Permanent Residence or Adjust Status

d. Copies of the intending immigrant’s passports

i. Biographical page of current passport

ii. Biographical page of invalidated passport

iii. Copy of F1 visa

iv. Copy of entry stamp

v. Copy of I-94 (front and back)

e. Copy of EAD, I-20s and Certificate of Completion issued by City College San Francisco as evidence of the intending immigrant’s F1 Student Status

f. Copy of the intending immigrant’s international birth certificate

g. Certified copy of certified marriage certificate

h. Form G-325A filled out by the intending immigrant, signed and dated

i. Two passport-type color photos of the intending immigrant

j. Form I-693 for intending immigrant seeking adjustment of status & Supplemental Form to I-693 Adjustment of Status Applicant’s Documentation of Immunization (in sealed envelope)

k. Form I-864 Affidavit of Support

i. Copies of Federal individual income tax returns for 2006, 2005 and 2004 along with every Form W-2 and Form 1099 that relates to the submitted returns

ii. Recent Bank Statement

iii. Pay stubs from the last six months

iv. Employer letter

Two more things. 1. Item #14 on your list should say I-693 A (I'm sure that was just a typo) and 2. if your husband is the alone sponsor, you do not have to submit proof of his US citizenship with the I-864.

Good luck.
