If my fiance did not have his vaciantions and will get them here in USA will the civil surgeon have the form for him to fill out? or do we download it and print and take to the civil surgeon? Since he will not need a medical does he fill out

If he had his K-1 medical within 1 year, he will only need to get whatever vaccs he requires. The form is I-693A. The civil surgeon will have the form and they will advise him of which shots he needs. My wife only needed 2. MMR and Tetanus. Also, see if your fiance can get an english copy of his vaccination record from the hospital he got his K-1 medical done at before he comes to the US. Then they will transcribe that data, and any additional shots he needs, to the I-693a.
