Greetings,

I have a friend who has developed a medical condition (fibroids) who may require surgery. Can she overstay to have the surgery here? If so, how do we go about that process? Sorry if this is in another chain already but I couldn't find anything.

You can request to extend the length of stay in the US. You can file this with USCIS. I believe it is form  I 539

Is she in a position to travel to have surgery in her home country where she may have medical insurance?  

Overstaying for routine medical needs is not advised as an extension denial..whcih can take several months.. may risk her visa for future travels to the US.

First thing, has she discussed the issue with her Travel Insurers, usually they would focus on repatriation if this is possible due to US medical cost, an Air Ambulance would be cheaper than any sort of stay in a US Hospital.
