K1 visa - Form DS 160 - Sydney

Hi all:

 

i have few questions on form DS-160, just wonder if some of you can shine some lights on this.

1. in the form, it ask for National ID number, in Australia, we don't have national ID number, we use driver's license as ID number, do i just put driver's license ID?

2. It also asked if I have ever been issued a US visa, I applied the visa waver program online does this count as a visa?

3. It also asked: do you have documentation to establish that you have received vaccinations in accordance with US law? - what kind of documentation and which vaccinations they are refer to there?  doctor letters?

4. in personal information 2 - country / region of origin (nationality), I assume I need to put my country of birth in this cell even though I am Australia citizen. is that right?

5. in the name section, i have a legal name, also i have an English name (which my friends and colleagues know me of), i assume i need to put this English name in the form as well?

1  we don't have a national ID either so put N/A / do not leave a section blank

2   you applied ?   Did you get the visa?

4  it your birth country different than Australia?

5   name is any name you have legally used to do business

For question 1 I would leave it blank if you have already provided a passport number elsewhere.

 

2. No, you didn't have a visa if you've used the visa waiver program. The VWP is exactly the opposite of a visa. It is visa-free travel permission. 

 

3. I assume you are aware that you will be required to have a medical before the interview. There are certain vaccinations that are required for people intending to live in the USA. If you have not had them yet, you will be given them at the medical. 

 

4. Nationality and country of birth can be two different things. If it asks for nationality, give your nationality. If it asks fir county of birth, put the country of birth. Are you a dual citizen? 

 

5. You must provide your legal name on all forms, even if you are commonly known as something else. If the name your friends call you is not used for official matters, you don't need to provide it. Only names they have ever appeared on your passports, driving license, credit cards, bank accounts, health records, etc.

Married: September 27, 2014

I-130 filed: February 5, 2016

NOA1: February 8, 2016 Nebraska

NOA2: July 21, 2016

Interview: December 6, 2016 London

POE: December 19, 2016 Las Vegas

N-400 filed: September 30, 2019

Interview: March 22, 2021 Seattle

Oath: March 22, 2021 COVID-style same-day oath

 

Now a US citizen!

thanks for clarify these questions.

 

I originally from China (ie origin is china), but I have given up Chinese citizenship and became Australia citizenship. China doesn't allowed dual citizen.

so i assume i will fill the form with Origin is China.

 

also on the topic on name, i have a legal name which is used on all my legal documents, including passport, driver's license, medical card, bank statements etc. but i also have an English name which all my friends and colleagues know me of, i suppose i don't need to put this English name in the DS 160 form?
