Current Government documents?? RE: DS5535, Request for supplementary information

Has anyone seen a more current document than this one regarding the US government's request for supplementary information?

(i.e. the DS-5535 form that requests information about your social media, emails, family members, travel, etc.)

My understanding is that the May 2017 request and policy expired after 180 days (November, 2017). Has it been renewed? I can't seem to find any documents nor news since November that indicate either way.

Okay, I found something. Some help to interpret this? I'm confused about that January 31st expiration date.

the notice says investigation concluded in May and approved and expires 6 months later which is November

So. Next question. Has the government been giving out this form since the policy expired in November?

thanks for pointing out this thread, my wife got an IV additional information questionnaire in email having the same questions as the DS-5535 on Jan 23rd.
