Inexpensive I-693 doctor in Illinois recommendations?

Received my interview notice in the mail for February 12th today and I just realized that I forgot to do an I-693. A lot of doctors are charging $300+ for it, which is very expensive for me. I have had most of the vaccinations done, but my doctor back in the UK  cannot give me a record of them so I'm going to have to get all the vaccinations done again.

 

Can anyone recommend a doctor in the North West Illinois area? 

 

Also, what's a good price for the I-693 examination and the vaccinations to go along with it?

I'm from Wisconsin, Milwaukee area. In November 2017, we paid $340 for a full physical exam for immigration. That was expensive. I called a few clinics in the same area (I found a list of clinics on the uscis site), and a couple of them wanted $200+ for the exam. So, I suggest calling around and comparing prices. Just make sure that the price they tell you includes everything you're going to need (that's how we ended up paying more).

 

Also, I suggest looking into doing blood work to see what antibodies you have (as in, what vaccines you had received before). It might be cheaper than doing all of the vaccinations again. Bear in mind that getting a lot of shots at the same time may make you feel ill,  and some of the shots have to be done at certain intervals.

 

Finally, maybe someone from the UK will comment and give advice on how to get those vaccination records from your doctor. You were supposed to get a copy of the form DS-3025, with the box marked "complete" (if you did get all of the vaccines) or "incomplete" (if you didn't get all of the vaccines), but I understand some countries have difficulties providing those copies...

06/27/2018 - Interview scheduled. But we couldn't make it that day, so we called USCIS and then sent a letter to local office to re-schedule; it was granted.
08/29/2018 - Interview re-scheduled. Approved on the spot!!! They did not ask why we didn't apply for AOS right after getting married. Review in Milwaukee USCIS office submitted
09/06/2018 - Green card in hand! It took 279 days to get it, but could've been sooner if not for the RFIE and re-scheduling of interview.

6/10/2020 - I-751 filed. Biometrics were waived.
3/19/2021 - RFE
5/20/2021 - sent a reply for the RFE
6/29/2021 - case transferred to National Benefits Center
8/30/2022 - case status updated to "Ready to be scheduled for an interview".
12/07/2021 - divorce granted (we had been having issues and the marriage didn't survive). I asked to switch my petition to Divorce Waiver and sent the request off along with my reply to the RFE.
05/31/2023 - case was updated at some point to show that my biometrics were taken (they re-used the ones I provided at AOS) and on this date, status update to "Ready to be scheduled for an interview" again.
No more updates were made; I assume this case was approved as I received my citizenship.
 
Citizenship:
2/5/2024 - filed N400 online under the 5-year rule (with pending I-751).
2/9/2024 - Interview scheduled for the end of March! The fastest I ever got anything updated! Not sure if it's a combo interview or not. Following advice here, I uploaded a simple Word doc asking to waive the ROC interview or do a combo.
3/27/2024 - Interview. It was a combo interview of both ROC and 400-N. I passed the test, but a decision could not be made as the officer had to review my 2-inch-thick ROC case file.
4/8/2024 - online status got updated that an Oath Ceremony was scheduled and that they mailed me the notice! I got it in the mail 8 days later.
5/9/2024 - Oath Ceremony. I am a U.S. citizen!!!

Timeline

The original DS3025 is with your AOS file and unless that is not marked 'complete' you don't need to see a CS.
Wait and see if they accept it and if not they will ask you to see a CS.
Getting all those vaccs again is extremely unhealthy.
Some don't have a DS3025 copy to submit with the AOS and that does not mean you need a I-693 at all.
So wait for the interview.

Waiting to skip ROC and get 10 yr GC due to over 2 year while pending AOS
AOS APPROVED Oct. 4, 2017 * Green card in hand Oct 13, 2017 !!!!!

First K1 denied after 16 month of AP. Refiled. We are a couple since 2009. Not a sprint but a matter of endurance.

I emailed my doctor in England and this is what they replied in regards to my vaccinations. Would a doctor accept this email as evidence that I had the vaccinations? 

 

It was from an email address ending in @nhs.net which is the national health service in England. They hold all medical information for all UK citizens.

I seriously doubt that an email would be acceptable - I can send you emails from nhs.net - it is very easy to spoof the "from" address.  Did you already have a medical exam in the UK, or no exam yet?

No I haven’t had a medical exam in the UK yet and am not planning to visit the UK within the next year. 

 

Could I get a letter emailed to me from my doctor confirming that I’ve had those vaccinations?

I had a really hard time convincing my doctor to transfer vaccinations from a USCIS approved form... So, I'm thinking, no.

 

Ebunoluwa made a really good point: if they haven't given you an RFE for missing medical docs, you might just be fine without them. Wait until the interview and see what the officer says.

That is a general letter not a RFE and lists everything that normally should be brought along for a interview but K1 entries already
had a medical and vaccines in their country and IF THE ORIGINAL DS3025 IS MARKED COMPLETE then you wont need a I-693.
They have your original DS3025 as I said. Since you have no copy you don't know how it's marked so I repeat, wait until the interview.
If it is not marked complete they will tell you to get a I-693 then. Why bother now spending hundreds of dollars for nothing ?!

I think there is some confusion, OP is adjusting status and entered as a tourist - he has not had any medical exam yet.

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

Sorry, didn’t mention that. That’s correct, I entered as a tourist and haven’t had any medical examinations yet.

 

Is there an inexpensive test to check for antibodies I already have?

Health care is expensive in the US [...]  You want to ask them about the cost to do titer tests. Isn't it possible to get an official transcript of your medical file/vaccination log - photocopies and fedex are certainly cheaper than the tests.

Nope, I spoke to the receptionist at my doctors office and I was told they have no official transcript. They said they could only send me that email.

 

i noticed there are many different types of titer tests. How do I know which one I need?

 

Would having an official letter from my doctor confirming the vaccinations I had, scanned to me also work?

July 21, 2017 HOME VISIT.  Went well. Topic thread in AOS forum.
Waiting to skip ROC and get 10 yr GC due to over 2 year while pending AOS
AOS APPROVED Oct. 4, 2017 * Green card in hand Oct 13, 2017 !!!!!

First K1 denied after 16 month of AP. Refiled. We are a couple since 2009. Not a sprint but a matter of endurance.
