really? did you have to ask specifically for this- I wonder if my husband asked for the wrong one? his only has active illnesses on it. it says "patient summary- referral view" and it doesn't list anything he has had in the past including his bells palsy, depression, surgery, etc...

No I didn’t ask for it . I just said I need a gp summary and she said it will list all your medical detials and visits and if I was happy for that to be printed off and on display etc. I said yeah I don’t mind . And it goes back when I was born .

I haven’t got it yet tho she said i have to pick it up when it’s printed off . But she shown me on the computer what it will list

weird. my husbands is a one page document that just says he's been with them from 1999-now and his on-going major illnesses- they have also said if he wants more he will have to pay a fee. Was yours free you got? I am worried he hasn't asked for the right one or something as it doesn't show any of his history on it really besides the on going...

No I didn’t pay. She said it’s free . I just asked for a gp summary that’s all I asked for . So I don’t know what is right and what is wrong lol. It’s so complicated . It shouldn’t be this hard for us ! They need to make it easier.

Weird. It sounds very different to what my husband got.

You only need the flu shot for your adjustment of status. So if your AOS case comes up for interview (or approval without interview) when it is not flu season---you won't be required to have one. You might luck out with timing.

Thanks I'm just concerned my husband has the right paperwork. His summary says nothing about his history of depression, tonsilectimy or bells palsy so im a bit confused what to do about that.

 

If it were me i would be fine but he is not good with all this paperwork (hence why I'm doing it) and he hates going to the gp and is scared (terrified) of needles so of blood tests and vaccinations (Which he refused to get since he was a kid/teenager so we have had to pay for him to get them all now as an adult- I like to think it is karma)

There's only four shots. He can have Varicella waived by telling them he had chickenpox. No written proof needed. He can have flu shot waived if they mark it not available as I understand there is a shortage in the UK.  So two shots at the most.

Yeah, well since UK does not vaccinate for it, pretty much everybody has had chickenpox or lies about it and is easily believed.. With a DCF (spouse) case, the waiver "not routinely available" can be given because Varicella vaccines would be hard to find in the U.K.

He already got the flu shot. He had to get a tetanus shot and one other but I'm not sure which one. He still needed hepatitis a And b but there is a shortage.

My fiance is having a similar problem (he is from South Africa) he had his wisdom teeth out and a minor finger surgery and the hospital doesnt have records anymore. Our lawyer said this was fine and not uncommon, but he should still disclose the surgeries if asked! Can't do anything if the hospital and doctors no longer have records.

so for the vaccination records, I have

 I have also just had a new HEP A and B shot but it isn't listed as it was free from the Sex health center. 

Also having a BOOSTER TETANUS 2018

 

is there anything im missing 

 

it also states I had chickenpox in 1989 on my medical print out

My medical summary from my GP was missing some information too, but it wasn't an issue when I had my medical at Knightsbridge. I had surgery for a broken leg and spent a few days in hospital as a result back in 2002, and this was missing from my record. I explained this to the person conducting my medical (as they could see the scars), and if I recall correctly I had also listed it on the form I had to complete prior, and everything was fine. 

 

The only slight problem I had was that a vaccination I'd had as a kid was missing from my records. Rather than me paying the full amount to have the vaccination there and then (as I was convinced I'd already had it when I was younger), I paid for them to include an additional test as part of my blood test so they could determine if I'd had the vaccination (turned out I had). 

 

The doctor conducting my medical said it was common for things to be missing from patient records, and blamed it on bits of paper going missing as the old paper records were migrated over to computer files within the NHS around 10/15 years ago.
