Need help filing

I have an issue with my EAD card been returned undeliverable. I have been calling uscis all over for two months now. Today just called my congressman office for assistance and they sent me these paperwork below to fill. 

My questions are 

1/ on page 1 since it is for I-765 am I the petitioner/applicant and also the beneficiary?  Or is my husband (sponsor) considered as petioner/applicant?

 

 

2/ on page 2 what is the staff member? 

 

3/ on page 3. What is the federal agency involved? And what is the federal ID

This is just first step so congressman start following up for issue. This is just to document that u r allowing ur information to get shared with congressman.

1-information you should fill is your information congressman workers need your permission to look your case so not your husband fill your information.

2-staff member is the congressman worker who is looking for immigration cases

3-Federal agency is the USCIS
