Hi! Try this civil surgeon which I got from the civil surgeon list on the USCIS website...

Tel: (773)907-7017

His clinic is at Swedish Covenant Hospital. If you just need to transfer your medical history to your supplement form, when you call them, tell them that you just need to transfer your medical history to the supplement form for immigration purposes and that you don't need full medical exam according to immigrations (your K-1 medical exam is valid for 1 year) because they might tell you to get the full medical exam.

I went there last December and they charge me $25. My friend went there too last month and the amount is still the same but she had to pay extra $40 for 1 immunization that she lacks.

By the way, I have called almost all of the civil surgeon in Chicago area on the list from the USCIS website. Dr. Miff charges the lowest fee and the other doctors wants me to get full medical exam even though I kept on telling them that I only need supplement according to immigrations.

I found one in Rockford who charges for $25 so thats 45 minutes away as opposed to 2 hours in chicago. When i tried to explain to them what i need the nurse was like, "Don't the signatures have to be the same signature on your I-693A as your Vaccine Supplement?" These people seriously know nothing.

You do not need to make an appointment, you just walk right in. It may not be done asap, depending on how busy they are, but the nurse said you will get it that same day.
