Where do you get the medical history form necessary for the medical examination?

Timeline

One document on the list to be brought to the medical examination is a medical history form. How do I get this form? I recall filling one out and sending it to the NVC which would mean they are going to send it to the embassy so how do I get this form on the day of my medical exam?

This is probably long gone you immunization from will need to be updated with all the necessary shots (if you don't already have all of them)

Assuming that you haven't done so already but you will need to go to Andrews Memorial Hhospital at 27 Hope Road, Kingston 10 Jamaica, WI. Here are some info from their website

 

Before you begin, you must have:

  1. An appointment letter from the Embassy
  2. Medical History Form from your package
  3. Passport (valid for at least 6 months)
  4. 4 passport sized photos
  5. Immunization records if available
  6. Current medications
  7. Current address of where client will be living abroad. Please include: street number, street       name, city and area code.
  8. Date of birth of all your children (FOR MOTHERS ONLY)
  9. Local telephone contact in Jamaica (name and number)

We recommend that you budget an additional $30,000 per applicant for children under 5 years and $20,000 from 6 years to adulthood to cover the costs of vaccines or any other additional tests you may be requested to do.

Failure to complete your medical examination well in advance of your visa interview may result in delays with your visa application.

 

Once you have fulfill the medical requirements it will be sent to the embassy before your interview. You will also be handed a copy. Keep it, copy it, you will need it.
