As long as the complete box is checked and the medical exam is within one year you shouldn't need the I-693.  The letter with the instructions quoted is good.  Other options would be an infopass appointment or contacting your congressman, as previously stated.    I hope you get it all resolved.

When yo get a chance update your profile to filing for: Adjustment of Status.  Good Luck!

Oops, I forgot to do that a long time ago! Thanks for the heads up - I'll update it now.

Hi everyone, looking for some advice here please.

I had my green card interview on Tuesday, and today I received a letter stating that my I-693 hadn't been submitted and my vaccination records were incomplete. I'm a little confused here. I had all of my vaccinations done in the UK before I left, and the doctor signed and stamped my forms to say that this was the case. The sealed record was submitted as part of my K1, and as per the instructions here, I believe that I shouldn't have needed to send anything else in my AOS package:

If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form (sealing the original in an envelope and giving you a copy)

The letter from USCIS states that I have to see a civil surgeon and get him/her to sign and seal the I-693. Obviously, I'm very keen to avoid the cost of this after spending a small fortune in London to get everything done and ensure that all was in order. Did I do something wrong here, or is there something I need to do to avoid another doctor's fee?? NOTE: I did not submit an I-693, but instead included a copy of my signed record from the doctor in London (the original was sealed and taken from me) with the stamp saying that my vaccinations were all complete and I was ready for immigration.

Any advice appreciated, even if it's to tell me I made a dumb mistake! Thanks in advance.

We submitted a copy of the vaccine records when we filed AOS. Did you not do this?

Sorry for the late reply. I did, but I have recently found out that the problem is with the consulate... USCIS are saying that they never received my medical results from the consulate in London. So it would appear that the original has gone missing somewhere along the way, and all I'm left with is the photocopy that the office gave me, and USCIS won't accept this. Not really great news - I'm struggling to find a doctor who will transcribe the copy, so I may have to get a whole new one done and pay hundreds of dollars for their error!

They wouldn't typically have left the vaccine page out at the embassy. It's 1 of 4 pages of the medical report. The report would have been in your white envelope turned in at POE. More often somebody fails to order it out of storage when you file AOS or it was alphabetized wrong and they didn't look very hard for it.

Did they confirm that they have the other three pages of the medical file? If not, a transcription of only your immunisations won't be enough.  Did you try Knightsbridge to see if they retained a record of your full report? At one time they told me they do not keep anything, but that may have changed. They could mail a sealed envelope maybe??

They just said that my "medical report from the consulate was missing", so I'm not sure exactly what that entails... I'm assuming you're right about it being misplaced or alphabetized wrongly after POE. It was definitely in the sealed envelope at that point, because they let me in without any issues! Knightsbridge don't keep anything, unfortunately - looks like times haven't changed much there!

If I can get a civil surgeon to transcribe my vaccinations and fill in the relevant parts of the I-693, that will be enough as the K1 medical I had is still 'valid' under the rules on the USCIS instruction pages. I've called a few places today, and have some more to try tomorrow. It just drives me nuts that I'm having to do all of this and spend more money to fix someone else's laziness/mistake. But I guess we're all familiar with how frustrating this process can be by now ;)

Your K1 medical is not 'valid' because it doesn't exist for them to check. Disappeared. They must see a medical report, so you need to have a full exam at a civil surgeon who will fill out the entire I-693. The CS will accept your photocopy of immunisations from Knightsbridge or anything you have from your GP to fill out the vaccination part. You're going to need a current season flu shot so get that at any drug store or health department if cheaper than what the CS might charge. Ask prices at a few places.

You may have the same "$$#"%#" problem as I...Because they took 15 months to finally mail me the interview letter...my medical exam expired and now I need to see this civil doc, who wants to give me a ton of shots, draw blood for siphilis and gonorea (which i only could have gotten here in US,since they are non existent in my country in EU plus they already tested me a bit over a year a go fo this), do TB test (that horrible skin test) and then charge me for this "wonderful" experience 450$.

So it may not be that is incomplete,it may be expired if you had your medical over a year or so ago.

I'm fuming right now- just got the letter in my mail and yes...i'm less than excited to put my immune system under stress with ,let's see 5 different vaccines in 1 day!

You need to check on that because adults only need a maximum of four shots and you may have had some of them previously or had chickenpox to get the Varicella shot waived. Do you have immunisation records from your home doctor or your visa medical (DS-3025)?

I just came from there and they gave me 3 shots even tho I HAD ALL OF RECORDS with me. I'm beyond pissed. It's not just 450$ they made me lighter and if measels tests comes back that i have too low titer they'll give me that shot ,too, and lighten me up for another 125$.

I already have a terrible headache...am just waiting for fever and urticaria to follow within next few hours.

So to everyone else... be prepared to pay and mentally prepare for all those shots and take a day or so off,coz specially MMR vaccine gives you headache, muscle and joint pain, nausea ,vomiting. If I idn't take Zofran I'd be throwing up already. Just posting this so anyone else going through this can be prepared.
