I made a copy of the whole 4 pages of the G35a form. All 4 pages look exactly the same. I checked the pdf file to be sure that it wasnt a printing error but it's the same from where you download it. Are all 4 pages meant to be the same?

Yeah, that's the way it's supposed to print out. You have to send ALL 4 copies in with your AOS Package.
