Vaccination record question

Hi. I got a varicella shot at the medical for the k1 interview, and the lady (nurse? doctor? i dunno! she just gave me the shot :P) told me that I need to get the second shot on April. Heh, April's almost over, and I've yet to get the shot because I've been worried about one thing - where do they write that I DID get the second shot?

Are other doctors allowed to write on the sheet of paper given after the medical (the ds3025)? Or will a doctor's certificate suffice?

Unfortunately, the clinic where I'm getting the shot doesn't have those.. cardstock thingies where they write down vaccination records (My fiance suggested this).. though they ARE willing to provide me with a doctor's certificate (I'm assuming this is just something they write down on paper, but I'm not sure)

PS: I'm still in Manila, so I can't get the second shot done by the surgeon person who's supposed to transcribe my records into the I-693a.. I guess the question really is: Will they honor a doctor's certificate regarding the shot from here?

Only USCIS accredited civil surgeons are authorized to transcribe onto the I-693 Vacc Supplement, duly signed and sealed in an envelope. The immunization shots can be administered by any other clinic/hospital/public health dep't.

I did call, but the people there aren't very helpful. All I got was "It (the vaccine) can be obtained at any outside clinic." :unsure: Finding a place where the vaccine could be administered took me a long time (always got an "I'm sorry, but it's not available here"), but now that I have, they don't provide those vaccination cards.

I guess the DS form is off-limits to non civil surgeons as well, huh? :(

Timeline

A doctor's statement that you have received the booster shot for the varicella should be fine. Basically, you just want proof - which means documentation of some kind - that you did receive the shot. When you get to the States and are getting the vaccination supplement completed just bring all of the different types of 'proof' with you to the doctor. If any of them are not in English, then arrange to have them translated into English so the doctor knows what they say. He/she will be able to transfer the information from your different type of documents to the official form required by immigration. I used a combination of school vaccination records, an international health vaccination record and childhood vaccination records along with the vaccination worksheet provided from my medical in Canada. The panel physician here actually chose not to use the vaccination worksheet because she couldn't make out the doctor's handwriting!
