Hi,

The instructions to the AOS say, that non field has to be left empty and that you MUST enter "none", "N/A" etc. there.

Well, the latest form does not allow you to enter this information at all the fields. So, a solution would be to print out

and add with wet ink those additional notes. However, the bar code on the form would be useless and would actually

only indicate the "typed on computer" fields when scanning. Please advice what to do.

There is no need for that. If you can't put it in with the computer don't worry. I only put n/a where the computer allowed me to and it was accepted without any problem. There were numerous empty boxes on my form. We tend to take the instructions very seriously because this is so important to us, but we are only required to put none or n/a where we can actually type something in if it is relevant. For example, I put n/a for the middle name box since my step-son has none but did not do anything in the boxes for his state or zip code since in China he has a Province and Postal Code. It was accepted without problem.
