it used to be I-864 but have recently updated and changed it, I am happy I noticed your question, as I left with my husband in Egypt the old form, so I have to fill out a new one and send it to him via facebook to print out.

such as the min income level?

I know that in the I-864  you didn't have to provide evidence of the amount you claim to make unless it was asked for. on the I-134 you must not submit evidence from bank, your employer, if your self employed a copy of your tax return.

I left my husband a copy of my last 3 years of tax returns. this I-134 says nothing about submitting tax returns unless your self employed, so now I am confused as to what they will accept.

The I-134 is for a fiancé visa.

The I-864 is for a spouse visa or for adjustment of status.

The most recent forms are always found on the USCIS site https://www.uscis.gov/forms

The OMB date is not an expiration. It is a date when the form is due for review by the Office of Management and Budget for review and clearance in accordance with the Paperwork Reduction Act of 1995.
