Card not received after AOS approval

Hello All,

 

I received a notification that my AOS has been approved, and that the Card has been ordered.

In addition, on November 7th, I received another notification that the Card has been sent.

However, till now I have only received an approval notice I-797 Notice of Action, saying:
"The above application has been approved. Prior to receiving your card...."
But I have not received the Card since then. I noticed that I-797 Notice of Action was sent to my old address and forwarded to my new address with a yellow sticker by USPS. Note also that I have changed my address at USCIS and at the post office.   Also, the notices to appear for interview and discovery was sent to my new address, I am not sure why the last mail was sent to the old address. I have phoned USICS, and they asked me to wait for 30 days before requesting a new card to be sent. Question do I need to file new 1-90 to have the replacement started? This is because my wife who received the  I-797 Notice of Action approval notice said the envelope was half opened. At this point, we are not sure if the envelope contains the Card and it fell off or something, hence the card might not get back USCIS. A lot has been going through our heads. Please advice.
