How have her circumstances changed since denial two years back? 

I-751 journey

It's not clear to me exactly what happened. Was the photograph the only objection? Did she actually have a proper interview or did the CO simply tell her she needed a new photo before he could interview her? (I don't understand why else they would they ask her to come back to interview again).

Aren't photograph(s) taken at biometrics along with fingerprints for pak applicants? ..by trained staff

From the last time I did B2, admittedly a while ago, you needed to both supply your own photo and have the bio photo done at interview.

Things, I believe have changed. When we did biometrics (for CR1), it was done in the days leading up to the interview at a different location; NIV applicants were biometric-ing at the same time. No pictures were taken at the consulate.

That doesn't change the fact that the applicant has to upload a photo with DS160 and still have a biometric one done, whether at the consulate or a facility approved by the consulate. (I just checked the DoS pages and the photo upload with DS160 is still required.)

 

(In any case I rather suspect that where the bio photo is done is dependent on country.  Our photos for UK visa here were done through the USCIS ACS ...I guarantee you that does not happen in our home country ;) ...and I don't think even now that the US outsources it where I come from)
