I'll reiterate what others have said...a large age gap alone is not a reason for a denial. If the age gap is uncommon and combined with other factors, then it can be (part of) an issue leading to a denial.

I highly doubt they would deny someone based on age difference alone. There would have to be other reason like the ones stated above. Mainly proof of a genuine relationship but age gap...no. Love doesn't discriminate against age.

From what I have read when talking about a large age gap especially young foreign male and older US female where the male is from a MENA country the couple has to add a lot more evidence to show the relationship is real.  More in person visits, more commingling of a marriage if going for a CR1, etc.  As others have said, age alone cannot be the reason for denial, but a large age gap as I described above certainly gives a CO pause, and the burden of proof of the relationship is on the couple.  It is certainly not impossible, but in reality it does tend to be more difficult.
