My girlfriend is trying to pay for her visa. This is her 2nd failed attempt in a span of 3 weeks.

There is more than enough money in her account. 

This is NOT a credit card it is a visa debit card issued by her bank and attached to her account.

We went to several banks yesterday to attempt a transfer to the embassy from her account. The price was 136 euro. We went to probably 6 different banks, o those that could help us they all said that they either could send 135 euro or 140 euro, not 136. 

So we tried again online to use her card, which has way more than 136 euro, but we were once again greeted with "Your transaction could not be accepted at this time. Please retry or use an alternative card."

Any idea why? We just used the card as recently as yesterday and experienced no problems.

First thing you need to do is contact your girlfriend's bank to see whether they got the request for the debit and, if so, whether they declined the transaction.  My husband was in Brazil recently and was denied twice, (at the same merchant) when he tried to use his credit card.  I contacted the credit card company, and they were able to determine that they never even got a request from the merchant.  It turns out the merchant did not accept "international" credit cards.  It's impossible to know the cause of the denial if you don't know which side of the transaction issued the decline.
