Hello everyone! Hope somebody can help me with this... When I was born, my parents were not together and I had only my mother’s last name. Then by a judge order my dad had to give me his last name and my birth certificate has what I think it is an amendment, stating that this new one supersedes the previous one. I have only used one name.
My question is, is this considered a change of name? I understand it is not since my legal full name (the one that is on every document I have ever used) is on my birth certificate, its just that has that note in the back.
And if so, that note would be enough to demonstrate what happened there? 

Also, should I fill out the part that says “other names used”?
