Need some help here please....

My Wife and I have an interview at USCIS for her green card from a KS1 Visa next month. We were looking through the checklist and saw I-693. When my wife was in her home country she got chest xray, blood taken, but remembers not getting any shots/vaccines. She got two manila folders after the appt one green and one white. The white one contains her chest xray the other was sealed and she did not know what it contains. At her K1 Visa interview they took the green folder from her and had her keep the Xray folder. She has no i_693 form or DS 3025... Does she need to get a medical exam before our interview for the Green card? Or is this already on file with USCIS? This is under a year from her upcoming interview to when she got the exam.. Would really appreciate any help. Thank you.

I just called a civil surgeon to make an appt and they said we would have to do the bloodwork and the xrays all over again for immunizations because we don't know what she needs.. I told them that I don't have these records since USCIS took them at the point of entry. What on earth am i supposed to do???? This doens't make sense to me.

I'd recommend taking a look at this thread on the topic:

The I-693 you get here: https://www.uscis.gov/i-693

And for reference here's a copy of the DS-3025: https://photos.state.gov/libraries/thailand/591452/iv/ds3025.pdf

As per the instructions in the I-693, you don't have to submit I-693 if your filing within a year of her k1 medical; all you would need to submit is the DS-3025, which they should have given her a copy when picking up her K1 medical results in the sealed folder.  In your situation, I'd try giving that original medical office a call and see if they can't email her/fax her a copy of the DS 3025 (if they still have her file?). May save you some trouble.

If not, phone around and see if you can't find a CS that will transcribe just her vaccines (so long as she has her childhood vaccination records).  If she's missing any, they do unfortunately need to do the blood test again to confirm immunity. With the I-693 some doctors aren't aware of the K1 medical and will stress redo' ing the whole thing, but don't listen. The vacc's she'll need are: MMR-Mumps/Measels/Rubella , Meningitis, Polio, Tdap- Tetanus/Diphtheria/Pertussius, Influenza (only if medical is done in season), Hep A+B,  and Varicella ie chickenpox (if not contracted when a child). Depending on age they also require Pneumonia vax if over 65, and Rotavirus for youth. The DS3025 shows the list.

There's also lots of info on the forums if you search K1 I-693. :)

Hope that helps, it's quite the process!
