Spouses Maiden name on i751?

Hi, 

 

So I am due to submit my i751 next month and have a question based on the following: 

My wife changed her name to my last name recently which does not match her maiden name on the original application for adjustment of status or more than a few bills, mortgage statement, title and other documents. My wife has to put her current last name (aka my last name) but where would she put her maiden name as there is no space for it. There is however space to the applicant to put previous known names.

If she changed her name then her maiden name is one of her previous names.

Yes but there is no option for her Maiden name on the application form in Part 3 or 4.

There is space for previous names. In all other areas she uses her current, legal name.

In what part do you see an option for her to add her maiden name? I am looking at the application now, she is the sponsor and I am the applicant so her information goes in Part 4.

Aha, sorry, I thought she was the applicant and you asked on her behalf. 

 

I don't think you need to add her maiden name anywhere, just go with her legal name. If she took your surname they will notice that. You also add a copy of her state ID/drivers license that shows her new name, as well as your license/ID of course as proof that you are living on the same address. You can even add a little explanation on a separate sheet to let them know of her name change if you want to. Could even add a copy of her SS card that shows her new name, but same SSN as she had before the name change of course.
