Hi,
First, I wanna make sure that I have all the required documents for AOS before filing it, please notify me if I'm messing thing:
1. 2 Passport size photos 2" x 2"
2. Photocopy of my birth certificate
3. Photocopy of my passport info page
4. Photocopy of K1 visa page with the entry stamp in my passport
5. Photocopy of the marriage certificate
6. I-485 form
7. G-325A form
8. I-94 (Printed from the Official Website)
9. I-864 form
10. I-693 form
11. Cheque  ($1225)

Second, regarding the I-693 form I did my medical exam for the K1 process in January 2017 (I didn't pass the year!) & to fill the Civil Surgeon section in I-693 form I need to go to one who's designated by USCIS in my area, what are the procedures that apply in my condition:
1. Would I need a new full exam or not?
2. Would be helpful bringing the Vaccination Documentation Worksheet (Expired in Sep 30 2017) to the CS?
3. Is there fixed fee for the CS in my case? If no, what's the average or approximately fee?
If there's more details that you can provide to help me & to enlighten me I'll appreciate that :)

Thank you in advance...

g-325 A is obsolete. I-765 and I-131 will let you work and travel out of the country before you receive your GC.

The actual I-693 instructions state you do not need another medical if you had yours overseas within a year of filing for AOS. This gets asked about a lot here on VJ. You can do a search here to find more threads about the topic. Also, there is an AOS forum too: http://www.visajourney.com/forums/forum/7-adjustment-of-status-green-card-from-family-based-visas/

You do not need a medical, and you may not need to see a civil surgeon at all. If you had all the required vaccinations at the time of your medical and they are all marked off on your DS-3025, then skip the civil surgeon and just send in a copy of your DS-3025. Here are details of how to know if your DS-3025 is ok to send alone or not : http://www.visajourney.com/forums/topic/335754-ds-3025-complete-or-not/page__p__4971318#entry4971318

See above regarding I-693.

just in case:

*make sure to file the most recent version of the form - download it here: https://www.uscis.gov/i-485

* make sure to add the requested supporting documents for the i-864 (tax return, W2/1099 if required; verification of employment/pay stub can't hurt) - https://www.uscis.gov/i-864

* when putting the package together, I'd put the check on/near the top, as well as form i-485

* form i-1145 will allow you to get text message updates once they accept your petition

* second the i-765 and i-131 filing - you never know how long it'll take for AOS to get processed and if there's an emergency. those are free to file when AOS is in process.
