What part of I-693 do I complete?

Hi, arranging AOS from K1 visa.

 

I require a I-693 for my vaccinations, which I know both I and a CS need to complete.

 

The form says the CS must complete any section from 4-9 and that I must complete section 1.

 

The instructions say I should complete section 1, the CS will do everything else.

 

Who completes section 2 and 3 of the form?

When did you get your K-1 medical?

 

Was the DS-3025 not marked complete?

 

If so and it's been under a year then you will not need the I-693

My medical was a few months ago but my Td was out of date so DS-3025 was incomplete. I had that done just before I travelled to the US. 

 

So just getting a transcription from a CS for all my vaccines.

 

But the I-693 instructions do not match the I-693 form as per my original post.

2. You fill out and sign in front of the civil surgeon.

3. Is about your interpreter.if you don't have one, then 3 is blank.
