Hello everyone,

 

Firstly I'd like to say thanks to everyone who contributes to these forums as it has been a major help to my wife and I along our journey. Luckily we have not needed to ask any questions until now as everything has gone pretty smoothly!

 

We attended our AOS interview on Friday 20th October. Everything seemed to go well and the interviewer was nice and friendly.

 

Fast forward to today Wednesday 8th November and I have recieved a letter from USCIS stating:

The form I-693, Report of Medical Examination and Vaccination Record, that you submitted is incomplete. We need additional medical information to complete your case.

Page 4 of Form I-693 Report of Medical Examination and Vaccination Record is incomplete. Please submit a completed page 4.

 

My original medical examination was completed in London on Friday 23rd September 2016. I was missing booster shots for TD and MMR which were waived to be completed at a later date. I completed the missing shots as soon as I arrived to the US and then went along to a USCIS certified civil surgeon to have the updated transscript completed and form I-693 filled out as part of a Vaccination assessment and sealed in an envelope. (Which i also have a copy of sent to my email).

 

I am looking at page 4 now, and all is present apart from the fact that section 5 has not been completed at all. However I think that this is correct as the instructions in part 9 (vaccination record) of the form state:

For applicants who only require a vaccination assesment: Submit only this page with part 1, Part 2, Part 3, Part 4, and Part 6. of Form I-693.

 

I have gone over and checked all of those parts, especially page 4 which is deemed to be missing information. And everything required is completed. We also submitted the original worksheet completed by the surgeon in London from the medical exam.

 

I am wondering if this is a case of the IO not realising that Part 5 is not required, but has just seen a chunk not filled out and instantly thought that we/the surgeon had missed out some information. I'm really not quite sure what the best course of action to take here is now. Maybe there is an appointment I can make to verify what exactly is missing from page 4, as that information from USCIS is very vague to say the least! I really don't think having to go through a whole new medical is necessary, but of course if it means gaining my green card I would be willing to swallow it as a last resort.

 

Any information or personal experience you can provide would be appreciated so much.

I'm sorry that I can't provide personal experience. Having said that, it sounds that you are right in this case.

I would try scheduling an infopass appointment. There are not always helpful but if you're lucky, you might get someone who actually gives you information on your case. Sending an explanation without the completed page might result in another RFE from USCIS.

01/19/2018 Interview - recommended for approval

01/22/2018 New card is being produced :dance:

I've gone ahead and managed to schedule an infopass appointment in 2 weeks time. Hopefully the officer will understand what's happened.

Does it say anywhere who requires only an assessment? You didn't just have an assessment right? Since you also had the shots?

That's a good point. Although I'm not sure if the additional shots would count as an exam as they were not administered by the civil surgeon doing the vaccination assessment, but instead carried out at a standard local health clinic. Hopefully the officer at my infopass appointment can shed some light on this.

Either way I would hope that the USCIS approved civil surgeon would already know which parts of the form he/she is required to complete in any given situation, especially as this RFE is for a page that was entirely theirs to fill.

Hi all,

 

Just a quick post in case anyone comes across this in future, and maybe has an issue similar to ours. My wife and I attended our InfoPass appointment last Friday and after speaking with an officer, our file was retrieved and looked at by a supervisor. After around 15mins the officer returned and informed us that it was indeed an error on the part of USCIS and that we could disregard the RFE.

 

Today I'm pleased to say I received my Green Card in the post! Thank you to the contributors of this forum and for the replies to my post on this topic.
