01 - green card employment authorisation

Hi folks, 

Im applying for adjustment of status and am on form i-765 (application for employment authorization) in section 11, it asks if I've ever applied for employment authorisation before. I have had an O1 visa that allowed me to work for CBS studios (my then employer), they got me the visa in fact. Would I therefore answer 'YES" even though i have never had a 'WORK PERMIT CARD' or applied for a green card before? just a visa in my passport.

Not sure what Section 11 means. Are you looking at the current form? I think the question you are referring to is 15. This has another question directly below it "Which USCIS Office?". I am adjusting from E-2 status which is similar to your O-1, but I answered No. I think No is the right answer because your visa was issued by a consulate, you did not apply directly for work authorization with USCIS (it was implied by your Temporary Worker class).
