My husband had his medical exam in Montreal, and on the day of his interview with the embassy for the K-1 visa, was given the sealed packet of medical records from Medisys.  He was not given any separate forms such as DS 3025.  Now we are preparing his I-765 and realize he's likely going to need this phantom document he was not provided.  Is this normal?  Do we need to obtain this DS 3025 prior to submitting the I-765?  Note that per the instructions on the envelope and by the embassy, the sealed packet was given to the gentleman at the border upon entry to the U.S. and no other documents were provided back to us except for the certified copy of my divorce decree.

You usually include the ds3025 with the i-485 which is the adjustment of status application. The ds-3025 is the vaccination form and if your husband has had all the vaccinations the doctor would mark the form as "complete (requesting blanket waiver)"

 

Now if you do not have this, I believe you can use the i-693 form which is the medical exam report. You will need to have this filled up by a civil surgeon (and he will need to examine your husband to do this).

Normally from Montreal the DS-3025 is located on the OUTSIDE of the do not open package or given to you withe the CD of the xrays.
