Tax Transcript in Maiden Name?

Hi, my wife (who came here from the Philippines 5 years ago) and I have always been filing our taxes jointly her married name.  Her Social Security card has always been in her married name as well.

 

Yet, and this puzzles me, I just now noticed that the IRS printed Tax Return Transcript for each of the 2015 and 2016 tax years, have her maiden name as her name instead of her married name, even though we never used her maiden name when filing, and never used her maiden name when updating her status with the SSA.

 

Furthermore, the IRS printed Tax Transcripts from 2013 and 2014 DO have her married name. So for some reason it was changed in 2015 even though we did not change to using her  maiden name on our returns or with the SSA.

 

Is this a new policy with the IRS?  Can someone check their tax transcripts from the past year and see if they used madden names instead of married names?

 

If not, is this a problem we need to get corrected?  If so, how?

That's odd but if she came here with married name, got her SSN with married name, where does IRS get the maiden name? Is her maiden last name now her middle name?

 

I just checked our tax transcripts for the past 4 years, all show my wife's married name.

My wife came here on a K-1 VISA in her maiden name.  But she applied for a Social Security card after we got married, in her married name, as well as filed all taxes.  Yes, her maiden last name is now her middle name (which is how she used her name when filing with the SSA and IRS).  So I have no idea how the SSA or IRS got her maiden name.  We will go to the SSA next week to get it sorted out.
