Adjustment of Status - Procedure for K2 (child)

We are presently filing for AOS (Adjustment of Status) after being married under a K1 visa. My wife has a child of 10 years of age that traveled with her as part of the K1 visa.

My questions refer to completing the AOS paperwork for the child.

1) Filing a I-485 (AOS form) on the basis of a K1 visa, I need to file one for my wife, BUT do I also need to file a separate I-485 for the child? Two separate forms? Or is the child included as part of the mothers I-485?

2) Do we need to file a separate G-325A (Biographical Form) for the child, OR is she included with the mothers G-325A?

3) What name do we use for the AOS I-485 paperwork for my wife now that we are married? When we applied for our marriage LICENSE and CERTIFICATE, we used my wife's maiden name, can we now use her MARRIED name on the AOS, or must we continue to use the MAIDEN NAME, until or if she decides to change it? (We weren't sure what to use when applying for the license, so as not to mess up the AOS paperwork, but it has only confused everything regardless).

ALSO, we used her MAIDEN NAME when she had her physical for the AOS, we were told we needed to decide then which name to use then, for that was the basis for the AOS paperwork. We weren't married yet and used her maiden name. We hope we haven't messed up again.

Hi,

I can answer one question, the other ones I don't know. I also am applying for AOS with my 11 year old daughter. You have to file a separate I-485 for the child. There are some helpful answers to questions in the guides.

I found out that the G325a is only for children and adults over 14years old. But for every other form you fill out with the AOS, you need to do one for the child.

3) Apply in the name that your wife is currently using, which hopefully is the name that she wants to continue using and have on her green card.
