We filed for an AOS back in March. I recently received a letter requesting that my wife get an I-693. First, I thought that an I-693 isn't needed for K-1 filers. So, why the request? Second, my wife says that she doesn't have any immunization records with her. I believe that they are needed for the I-693. I assume that the immunization records were needed to get her K-1 Visa. If so, can I request a copy? My wife is from China. If anyone has any help, it would be greatly appreciated.

Hopefully this is not dated:

My experience:

I submitted a photocopy of DS3025 only, as stated in the I693 instructions.

The photocopy was mailed to me by the overseas doctor when I applied for K1 and got all the required vaccinations from the doctor. I did not request DS3025, so I was surprised when I received it, which turned out to be useful.

(I assumed the doctor sent the original DS3025 together with other documents to the consulate who then passed it to USCIS using the yellow envelope carried by K1 visa holder when POE. So we did not need to submit any medical related documents. Clearly I was wrong.)

The medical report (DS-3025) which consists of medical exam and immunization page from hospital in China is valid for one year for AOS. Did you send in any DS-3025 with the AOS package ? When you say your wife don't have any medical copy from medicals done for K-1 visa then I don't think you have sent in the DS-3025 but If yes, then there are two things to see here:

1. I guess the reason why they are asking for I-693 is because you only sent in the copy of DS-3025 thinking she won't need a I-693 for AOS.

2. According to your wife's VJ timeline she got her visa approved on "10-24-2016" and so her DS-3025 is due to expire in about 15 days from now and if you don't have a DS-3025 then you will have to do the complete medical exam along with the required immunizations.

I didn't send the DS-3025 in with the AOS package. My wife can't find the DS-3025. She should have received the DS-3025 form from the medical exam?

Actually, I just found out that she wasn't able to find her immunization records in China. That explains why she didn't get a DS-3025. I guess she'll have to get re-immunized. :(

When she got her medicals done in China, she has received a sealed package for Immigration at POE and she also received a applicant's copy because according to the immigration rules the applicant should also be issued with a copy of the medical exam. Since you haven't send in the I-693 then you should do the complete I-693 which includes the physical exam too.
