Hello! A week ago I sent my aos package and today I got it back because I sent outdated version of I-485. They opened medical exam envelope and messed up everything. My marriage certificate is stapled with cover letters  and my husbands birth certificate etc. They stapled my vaccination sheet with my husbands paystubs... When I sent the package everything was organized but they stapled documents that are not even connected to each other. Do I have to unstaple it all and organize it again or I should just leave it like this? Plus I sent 6 photos (2 for I-131, 2 for I-765 and 2 for I-485) and they put 4 pictures in one bag and on one picture it says I-485 and they put one picture in the other bag and its written I-131 on it. But what about I-765 photos and why they put 4 pics for I-485? And what about the medical exam? They opened it and stapled the envelope with the form I-693. I'm really confused.. What do I do? Please someone help!

Unfortunately no, I only have a rejection notice (I-797C) saying that I sent an outdated form and I need to resubmit my application on the current version of the form. There is also a green paper saying that I should place it on top of my application when I send it. That's it. They never said anything about if I should leave it as it is or re-organize it. They way they stapled things doesn't make sense and I have no idea what to do about the medical exam envelope. :(

When I applied N400, and they rejected me for some reasons, they stapled everything together... 

When I resubmitted, I made sure that my error is fixed, and sent the whole packages to them again without doing anything.

For example, you used the old version, you might try to fill out the new version, then placed the Green sheet in front of the package, then resend to them. I suggest Do not un-staple then organize. USCIS always has the way to deal with it.

They stapled all my passport page and visa page copies and all the copies of the birth certificate and translation with the I-485 form (even though they were supposed to be for I-131 and I-765 form as well). Less than a half of my husband's paystubs is stapled together while the other half of his paystubs is stapled in a different pile with my vaccination sheet. It all doesn't make sense. If i send it like this, won't they be even more confused? I just really don't know if they stapled it like that by mistake or it's supposed to be this way...

As I said, USCIS has the way to deal with this. I believe some other member could help you but I would not take a chance of un-staple then reorganize myself, in order to prevent any mistakes I made, or USCIS will have something to blame me.

Ok, thank you. I'm gonna send it the way it is but what do I do with the green paper? It says to place it on top of the application, but its stapled with the form I-797c on top of the green paper and its stapled with other documents as well. DOes it mean i have to unstaple it, take the green paper out and then staple it back or what? Because idk how else to put it on top of the application...

Pretty much a Yes. Take that green one out and placed in front of everything. 

I think you will be good to go. IF you are unsure, you could contact the Lockbox, they will reply your message kind of quick (3-7 days).

did you end up submitting a new medical exam document in a sealed envelope or sent the same opened medical back ?

Do exactly as the say, do not reorganize it as they started the process to organize it in required uscis format.  Anything you do will be undone.
