I hold a K-1 visa.

I have a full copy of my medical exam, which included a vaccination report by the doctor, and two new shots.

The original one is in my brown enveloppe, i assume, and thus I wont have that anymore.

So what do I do while still in belgium to prepare for the vaccination document requirement in the AoS procedure?

Make sure to bring the copy of the vaccination record when you come to the USA. When it is time to file for AOS, you will find a civil surgeon in your area. The civil surgeon will then transfer the information from the vaccination record onto another document that will be sent in with the AOS.

we sent his vacc. supplement worksheet DS-3025 with my husband AOS and got lucky. best of luck!! just figured that if we got a RFE we would then spend the money for the civil surgeon!!!

Alexander- there is a civil surgeon in Houston that will transcribe the DS vaccination form to the I-693a for AOS for $15. When you are ready I can give you the phone number.

I have a question too. I never got my vaccinations at the panel physicians office who did my medical exam because I had them done by my DR ahead of time. Therefore, they never gave me any vaccination sheet or whatever. When I go to the medical exam for the AOS, will my vaccination record do in proving what vaccinations I have had?

Did you enter on a K1? If so, and it's within a year, you won't need an AOS exam, only a vaccination sheet.

What did you submit with your K1/AOS docs as far as a vacc sheet?

Are you saying that he didn't have to have another medical exam? Ahhh, I am so confused. I don't have any of those worksheets - they were never given to me! All I have is my vaccination/health record which has all given vaccinations in it.

no my husband hasn't seen any other doctor of any kind. we sent a copy of his DS-3025 that his family was able to go and pick up and send to us. the approved doctor in morocco had the vacc. worksheet DS-3025 wrote in english, and we just made a copy of it.. i can't say it will work for all of you but, we really didn't have the extra cash for another exam. I was told that alot of the civil surgeons have not had any updates and they will not put there name on anything without them doing the exam. i figured that if we got a RFE we would then spend the money. we took that chance, and we flow through the AOS process.

they say no other exam is needed if it's within 1 year of your last exam. Not sure but just looking at my husbands records, I am thinking he needs more shoots. i will have to go on friday and see what the county nurse thinks.

I am also thinking y would the U.S and the american counsulate allow our loved ones in the states without the correct vacc. however we did find a civil surgeon who said we wouldn't have any problems getting an appointment within 3 days of calling for one. thats another reason y we took the chance on the DS-3025!!! best of luck everyone!!

You're okay, Magnolia. Those worksheets aren't given to everyone. They are attached to your K1 file, which is supposed to follow you to the AOS process. At least that's my experience as a Canadian K1 beneficiary. I wasn't given copies of anything either. Did you submit a copy of your vaccination record?
