This link is going to be able to answer almost all of your questions:

https://uk.usembassy.gov/visas/fiancee-2/applying-for-the-visa/

On that page 2. Applying for the Visa, it talks about the steps you can take after you have received NOA2, your case has been sent to the NVC, and then after it leaves the NVC when it gets transferred to the embassy.  Once your case reaches the NVC you can call them (1-603-334-0700 or 1-603-334-0888) and get your LND number and you will need this to book your medical.  In general, the medical appointments are available about 2 weeks from when you call Knightsbridge but they might have dates available sooner.  Once you have your LND number from the NVC you will be able to track it's status here: https://ceac.state.gov/ceacstattracker/status.aspx You should not Submit your DS-160 until your status on CEAC changes to Ready as there have been members of the forum who have done this and then had to redo their DS160 again.  On CEAC you need to make sure you choose Immigrant Visa, enter your full LND number and then the Captcha and it will change status while at NVC and after your interview.  At the NVC stage it might says Received, In Transit, and Ready (once it reaches the embassy) and after the interview you are likely to see Ready, Administrative Processing, and Visa Issued.

Even though the steps outlined on Page 2 say to fill out the DS160, then gather supporting documents, then book the medical, and then book the interview after completing the medical, that's not necessarily the fastest way to complete the process.  Once you have your LND number you can call Knightsbridge and book your medical, this is what we did, and then we completed the DS160 as soon as CEAC said Ready, and then scheduled the interview.  We didn't wait until after the medical to schedule the interview and had no problems.  The important thing is that when you book the interview you need to make sure it's booked at least 5 working days, not counting weekends, holidays, and bank holidays, after the medical to make sure they have time to finish up the medical results and get them to the embassy before your interview.

If you go through each page it will talk about what items are needed for the medical, you can look at the vaccination chart under number 3. Medical Examination - Required Documents and the questionnaire that you have to fill out and take with you.  You will also need passport photos to take with you as well.  Once you arrive for the medical you will turn in the required documents and they will give you additional forms to fill out and then you'll have a series of tests and exams (blood, urine, chest x-ray, physical exam, reviewing your vaccination records, and reviewing your answers to the medical questionnaires).  You're going to need to order a copy of the UK Police Certificate to take with you to the medical and you can do that through ACRO which will take a few days to a few weeks to arrive after you order it.  Their website is here:  https://www.acro.police.uk/police_certificates.aspx

On page 4. Required Documents it lists everything that you will need to bring with you to the interview, some of it may not be applicable to your situation, but review that.  A couple of important things to note there are that you are required to bring the long form birth certificate with both parents names listed, if you don't have that then you're going to need to order a copy.  Your fiance will also need to send you a completed copy of the I-134 form along with some supporting documents for that (letter from employer, 2-3 paystubs, W-2s, tax transcripts).  In general, for London they will accept 1-2 pieces of supporting documents to go with the I-134.  You will also need additional passport photos for the embassy as well, separate from the ones you took to the medical.  It's also advisable to bring an updated Letter of Intent to Marry from your fiance, just like you submitted with the original I-129f application just with updated dates and signature.

Hope that helps!
