Dual Citizenship Application Questions

My wife got her US citizenship over the summer and is planning on getting her dual citizenship through the Philippine consular outreach when it travels into town next month.

 

Requirements include a filled out application and a submission of a PDF of the filled out application ahead of time via  email, along with supporting documents.   

 

We have some specific questions about some of the sections on the application.  Perhaps one who recently filled out an application can help us:

 

Question 2d asks for supporting document of name change.  Do we put "marriage certificate"? And does this mean we need to include the marriage certificate among the supporting documentation?  (It wasn't listed.)

 

Question 13b asks for mode of acquisition of foreign citizenship.  Do we put "US Naturalization Ceremony"?

 

Question 17 asks for Philippine permanent address.  She is living here in the US as a permanent US citizen, so what does she put?

 

Does she sign and date the application certification on page 2 now before sending the PDF via email, or leave that blank and sign at the consular outreach?

YES include a copy of the marriage certificate (must present original at the outreach event), it is listed on the Embassy website instructions.  I will include links

 

Question 13b asks for mode of acquisition of foreign citizenship.  Do we put "US Naturalization Ceremony"?

Seems you are using a different form that what the Embassy has posted.   Item 12 offers a box to check  NATURALIZATION.

 

Question 17 asks for Philippine permanent address.  She is living here in the US as a permanent US citizen, so what does she put?

U.S. address

 

 

Does she sign and date the application certification on page 2 now before sending the PDF via email, or leave that blank and sign at the consular outreach?

To be signed in front of the officiating officer, or notary (the way the form reads) .   My wife signed before, I never asked if she signed the form again later .. not a major deal if they wanted it signed again ;)

Application for reacquisition of Philippine citizenship

REQUIREMENTS:

Submit the original and one duplicate copy of the following required documents to apply for reacquisition of Philippine citizenship:

  1. One (1) original and one (1) photocopy of the duly-accomplished Dual Citizenship application form, typed or printed legibly in black or blue ink
  2. Original and Copy of Philippine Birth Certificate issued by the National Statistics Office (NSO) / Philippine Statistics Authority (original to be presented before the oath taking, when applying by mail)
  3. Copies of Philippine and U.S. Passports (originals to be presented before the oath taking, when applying by mail)
  4. Copy of Marriage Certificate/Contract, for married women (original to be presented before the oath taking, when applying by mail)
  5. Copy of U.S. Naturalization Certificate (original to be presented before the oath taking, when applying by mail)
  6. Four (4) colored photos, 2” x 2”, taken within six months before the date of application, showing a clear front view of applicant’s face, with a white background. No sleeveless attire. Blurred or low quality photos are not accepted.
  7. Processing fee of $50.00 (non-refundable), payable in cash or money order made payable to "Embassy of the Philippines" (or "Philippine Consulate General", if application is made at one of the Philippine Consulates General in the U.S.

Thanks Hank.  However, I am using the same application.  My question was about 13b, where it asks for "Mode of Aquisition of FOREIGN Citizenship".  Question 12 asks about initial PHILIPPINE. Citizenship.  Either way, you answered my question, as I will put Naturalization.

 

Regarding question 17, I am puzzled as to why I would put our US address when the question asks for the "PHILIPPINE Permanent Address".  The next question (question 18) asks for the US address.

 

Also, the D.C. Embassy and the Los Angeles Embassy, whom we are doing it through, apparently have differing documentation requirements:

Yes Marriage Certificate.   Copies are required and the original must be present at the outreach event also.   Don't forget to bring both the Philippine & U.S. passports and original Naturalization certificate, plus copies.

 

Question 13b asks for mode of acquisition of foreign citizenship.  Do we put "US Naturalization Ceremony"?

U.S. Naturalization

 

Question 17 asks for Philippine permanent address.  She is living here in the US as a permanent US citizen, so what does she put?

Her home (family) address in the Philippines if she still uses that addresses when in the Philippines.  

 

Does she sign and date the application certification on page 2 now before sending the PDF via email, or leave that blank and sign at the consular outreach?

(think I answered this one correctly before.. yeah!  I got one correct! :lol: )
