Question about I-693A (Exam)

I am waiting for my AOS and was told I need form I-693 and that I should have gotten that from the place I got my exam at. Well, I knew nothing about this form and dont have a copy. What does that mean? What exactly is it fro anyway. I really dont feel like getting a complete exam all over again. Does anyone have the number for the clinic in Berlin (the one on argentinische alee)?

You should have been given the DS3025 vaccination worksheet when you had your medical done.

If you do not have this ask the medical facility to fax it to you.

Then take this to a civil surgeon here and he will transfer the vaccination info to

the I693A form. Only civil surgeons can do that.

Some CS will insist on a complete medical but you DON'T need it unless it has been

more than 1 year from your last medical.

The fee to transfer varies so shop around. $10-$35 is reasonable.

You may also have an out of state CS transfer it per mail if the ones near you won't

cooperate.

You will get a sealed envelope for the AOS and one copy for yourself.

Just want to add if you have the copies of all your vacinations to date (date given is required), they will accept that instead of the worksheet. In fact my Civil Surgeon didn't want to transcribe off of a foreign Dr's worksheet but see the proof for himself. it takes 10 min tops. Easy form

I had my medical in Frankfurt and I did not get anything either.

What you can try is copy your entire vaccination records (Impass) and file it together with your application. This is what I was recommended to do and what I actually did. I did not receive any RFE and was approved in march.

Otherwise if you want to get this form, got to the list with civil surgeons and get it filled out by a doctor (with additional costs).

Somehow as far as I know hardly anyone from Germany got the DS3025.
