How to answer "what if" type of questions in I-130

Hello everyone...I'm filing I-130 for my wife. I'm a bit confused on how to answer those "what if" type of questions in I-130.

For example, part 1 item 3 "if the beneficiary is your brother/sister, are you related by adoption?" Since I'm filing for my wife, this question doesn't apply. Should I leave both boxes (yes/no) unchecked? Do I need to hand-write N/A there?

Part 2 item 2 "uscis online account number (if any)". I don't have such number. Should I leave it black or put N/A here?

Part 2 item 4c "middle name". If I don't have middle name, should I put N/A or None?

Part 2, "Other names used (if any)". I don't have other names, should I leave blank or type in N/A? Or None?

Part 2, items 12.a-13.b, "if you answered NO to item number 11...". My answer to item 11 is YES, should I leave 12.a-13.b blank? I actually cannot fill in items 12.a-13.b on PDF file.

Thank you!

I heard Family name should be all uppercase?

Leave everything blank

No, don't leave anything blank! The instructions clearly state to write "N/A" or "None" on questions that don't apply so do this. There are posts floating around of people getting RFEs for leaving blanks.

Leaving it blank is some bad bad advice, be sure to follow the instructions, sometimes there's issues filling it all out on the pdf form.

I had a pdf editor and was able to overlay text.

I believe you can use black ink, check the instructions.

Correct, you can use a mixture of printer and pen ink. I filled out most questions on the computer and filled out others with a pen after printing.

For reference here are the official I-130 instructions, https://www.uscis.gov/sites/default/files/files/form/i-130instr.pdf . Item 3 on page 4 says the following:

Quote

Answer all questions fully and accurately. If a question does not apply to you (for example, if you have never been married and the question asks, “Provide the name of your current spouse”), type or print “N/A,” unless otherwise directed. If your answer to a question which requires a numeric response is zero or none (for example, “How many children do you have” or “How many times have you departed the United States”), type or print “None,” unless otherwise directed.

So just to reiterate do not leave any blanks, please write "N/A" or "None".

Ugg!  The PDF form is designed for computer entry but hand writing is OK.  When using Adobe Reader to complete the form, there are several places where if a certain box is checked, a section of the form does not allow one to enter data on questions that were rendered not applicable by checking that box.  As such, it is certainly OK to leave those blank.

For numbers that have not been issued yet, I recommend entering zeros.  For no middle name the question is applicable, "None" is actually a name in more than one language.  Since the form no longer allows you to enter ---- simply type "no middle name".

Bear in mind that a question about a middle name is definitely applicable even if you have no middle name.  Do not answer other questions that are applicable with N/A when None is the correct answer.  When you have entered, so "No Children" in the first space of the section that allows entering information for up to 5 children, just leave everything else about children "blank".  Use this logic on all forms and you'll be fine.

Thank you all for your replies, really helpful!

How about the question itself with "if" condition?  Like the example in my original post, part 1 item 2, "if you are filing this petition for your child or parent..."? Literately I shouldn't answer this question as it is irrelevant. Besides, there are 4 checkbox, but no space for "N/A".

You already indicated at the top of the form that you are filing for a spouse, right?  Leave the if answers to the if question blank because they have already been rendered N/A by your X in the Spouse box.

I agree with Jorge and Mr Hanky strongly on this as well. I would not leave anything  blank. The instructions are rather clear on that point and all it takes is one bureaucratic individual to send your form to the back of the line.

I didn't answer it because of the original poster. I answered it because it ranks high on google for this question which is how I came to it and wanted to help anyone who may be later coming to the post and using it as a resource.
