So.. I was just looking at filling out my I-485, I got to the section where it asks me to fill out my given name on Page 3 of the I-485. Here it asks for my First and Middle names exactly as on the I-94, however there's no section for a middle name on my I-94. 

In order to fill this out I pulled out my digital I-94. It lists my first and middle name as my given name. 

 

Last/Surname: (Surname)
First (Given) Name: (Firstname) (Middlename)

 

Any suggestions as to how to fill this out correctly? Do I need the I-94 corrected? or just put the middle name in middle name box? or put first and middle names from I-94 in the given name box? (as this would be "exactly")

I'd say use the name that's on your passport and visa because they are in the correct formats. You will still have to send photocopy of your biographical page of your passport, visa with stamps and also I-94 so that should not be a problem and they will know about it and also I would suggest you to use the  additional pages on the end of the application to write like a "note" regarding this confusion.

You have your proper legal name on page 1.

They want to know exactly how your I-94 reads. If yours says JohnPaul, then put that on page 3.

Thanks. Really appreciate it. One last question for the other part of the forms where it ask for names do you put the first and middle as given (like on the i94) or first in given and middle in the middle column.

In future always give the actual given name when asked and the middle name if it asks that etc. This box (so far as I can make out) is just to cross reference with your I-94 to verify your entry/inspection.

I have the same 'problem'. I can't fit both my first name and middle name in the 'First Name' box. I'm contemplating of writing them in their respective boxes as it really should be the case.

 

LE: Sorry, checked again, form i-765 doesn't have the room for both names,  i-485 does. How did you write your name in the i-765 OP? Did yours fit? I wouldn't like to do them differently.

I had the same question when I filled in the form. My first and middle name were both shown as 'given names' on the I-94.

I put my middle name in the 'middle name' field in the I-485 form and there were no issues. Obviously not sure if this would always be the case, as every case is different, but in my particular experience it didn't cause any problems.

Hi, I came across this post on doing a search and see that others have had their First and Middle names put in the location on the I-94 where it states 'First (Given) Name'. Is this acceptable or will it cause issues further down the line at all?

 

Not sure if related, but I did have to be manually verified to get my SSN which took 49 days from start to finally receiving my card. When I went into the Social Security Office the clerk found me on her computer system twice, once under my correct name as seen on my passport, the second one my correct name but in the wrong order. She tried 4 different ways to rectify it but couldn't and said she would have to send photocopies of my paperwork to Homeland Security for them to sort out. That got sorted but not sure if it was I-94 related due to my middle name being on my I-94 after my first name... rather confused and not getting any concrete answers, not sure I want to start trying to change things on the form if it's not needed?
